Description: The Care Team provide a Nursing Service to patients in their own homes. The Service gives help and support to these patients during their illness, convalescence and reablement programmes and at the advanced stage of terminal illness. The Care Team Nursing Service should follow instructions given by the Clinical Director or her deputy. Role and Responsibilities: •Tracheostomy care: Manage secretions and use suctioning as required, Administration of medication as required, Observation for change in health status Perform an emergency tube change if required and Implementation of care plan and recording of nursing notes. •Participate in the planning, assessment, implementation and evaluation of nursing care to enable the patient to continue to be cared for at home. •Provide advice and support for the patient, their family and the healthcare assistants according to their needs. •Ensure continuity of patient care by communicating and liaising with medical or nursing colleagues, providing accurate and timely reports of any changes in the condition of the patient. •Keep the patient and their nominated next of kin up to date as to the patients ongoing medical condition. •Maintain accurate records in order to reduce the risk of errors as per An Bord Altranais Recording Clinical Practice Guidelines. •Administer medicinal preparations in accordance with An Bord Altranais guidelines and The Care Team policies and procedures. •Report any medication errors immediately to the patient’s GP, doctor on call or the Clinical Director of The Care Team. •Administer wound care in accordance with An Bord Altranais guidelines and The Care Team policies and procedures. •Use evidence-based practise to develop and maintain high quality patient care and ensure the continuous improvement of the nursing service. •Report verbally any accident, incident or near miss during or immediately following the shift, depending on the incident, to the patient’s GP, Community Palliative Care Team or Public Health Nurse and to follow this with written details. An Incident form should be completed at the earliest possible time and returned to The Care Team offices. •To adhere to a ‘no smoking’ on duty policy which must be observed at all times. •Follow Medication Management Guidelines as per An Bórd Altranais Medication Management Guidelines and The Care Team Policy. Working Hours: 10:00 am – 2:00 pm, 7 days a week. Location: Bray, Co Wicklow. Pay rate: €24 - €26/hour (depended on experience. Requirements: •Registered with Bord Altranais agus Cnáimhseachais na hÉireann, Nursing and Midwifery Board of Ireland (NMBI) active registration. Have two years’ experience post qualification. •Experience working with palliative patients (desirable. •Must have Tracheostomy experience. •Excellent communication skills. •Excellent interpersonal skills. •Ability to work on own initiative. •Ability to work under the direction of the Clinical Director or her deputy. •Proven ability to display good judgement.
